Chornic disease follow-up
At GMF Mercier, Nurse clinicians work in a close collaborative team with family physicians for the care of patients with chronic diseases. This available teaching and surveillance service is reserved for patients registered with a family doctor at GMF Mercier.
Patients who are in the care of the clinical nurses are mainly affected by chronic diseases such as:
- Diabetes
- Hypertension
- Dyslipidemia (cholesterol)
- Cognitive disorders (dementia)
- Chronic Obstructive Pulmonary Disease (chronic bronchitis, emphysema)
To be followed by a Nurse clinician, you must be registered and referred by your family doctor at GMF Mercier.
