This is the long term follow-up of a patient by a family doctor. During the care visit the family doctor becomes familiar with the patient’s state of health as well as his or her previous medical history. The doctor will then continue to follow the patient over the years, perform the periodic examinations required by the patient’s condition and address any health problems that may arise.
